Physician-referred Patients

ActiveRx® conducts an in-depth Functional, Behavioral and Lifestyle Assessment, which is used by our care providers to create an ongoing treatment plan. This assessment highlights potential areas of lifestyle improvement and insights into how these recommendations are best able to play into the patient’s ongoing recovery process.

Self-referred Patients

ActiveRx® typically provides the same in-depth Functional, Behavioral and Lifestyle Assessment that physician-referred patients receive. We recommend meeting with a member of our behavioral staff to discuss the meaning and implications of the assessment results and to determine what future steps may be taken.
